Student Information Access Release

Lake Forest Country Day School provides the parents & guardians of current LFCDS students access to Panther Portal Online Community, a password-protected Website providing general school information as well as individualized schedules and details about a parent’s/guardian’s student(s). Parents/guardians may request to allow others (“Grantee” below) access to the personalized information about their student(s) on the Panther Portal by completing this form & submitting it to the Administrative Assistant to the Head of Lower School or Upper School, as appropriate.
